At Touchstone, we offer comprehensive supports for youth into young adulthood. Transition Age Youth (TAY) services begin with transition planning at age 15 and services are available through age 25 at Touchstone to ensure a stable transition into adulthood. TAY services are available in conjunction with our other services based on individual member needs.
Touchstone aims to support independence and confidence by teaching crucial skills, providing evidence-based services, and partnering with the youth to honor their voice and choice, Our youth-oriented services are equipped to support the transition to adulthood as our members navigate school, work, mental health, housing, relationships, and independence.
We believe every young person deserves guidance, stability, and opportunity during this critical stage of life.

Family engagement when appropriate to support independence
TIP Young Adult Independence Program
TAY planning with formal reviews at least every 12 months
SMI (Serious Mental Illness) eligibility determination, if applicable

TIP Young Adult Independence Program
The young adult groups Touchstone offers follow the Transition to Independence Process (TIP), an evidence-based program that supports participants in developing crucial skills. The groups break down and work on different skills each of the 12-16 sessions, including topics like:
Health & wellness
Social & communication skills
Housing & independent living skills
Budgeting & money management
Job readiness and employment supports
Groups are available for ages 15-17, 18-21, and 22-25.
