School Behavioral Health Services Survey
You are being invited to complete this survey as a parent/guardian whose child/children were referred for behavioral health services through their school. This information will be used to improve the process for referrals and use of behavioral health services through schools. Unless you choose to include your personal information, your responses are anonymous.
Member Grievances
If you are dissatisfied with any aspect of services provided by Touchstone Health Services, please let us know. Touchstone Health Services are dedicated to providing services quickly, professionally and appropriately. We encourage all our member families to talk with their case manager to resolve any concerns. If a family prefers to speak to a supervisor, the case manager will provide that information. If there is a concern that cannot be resolved at that point, please contact our Compliance Department using this form.
